The following items must be submitted to the ELC office before a child can begin in our program:

• Student registration form

• Pre-Admission Form (DHR-CDC-739)

• Parent/Center Agreements

• Current immunization record

• $100 registration Fee (renewable each year)

• Tuition

• Lunch form

• Photography permission form

• Evacuation Plan
